• Important: For security reasons you are required to attach a copy of your passport (or other government issued ID, such as a driver’s license) to this form.

• The session fee will incur a 50% surcharge from the current listed prices on www.hegre.com/tantra/massages.

• A 50% deposit for the massage is payable in advance using credit card (the balance to be settled in cash at the end of the massage).

• All travel expenses, including flight tickets, airport transfers, accommodation (with WiFi) and meals are your responsibility and must be paid up-front.

• The session will always take place in the therapist’s hotel room.
